What insurances do you accept?

We accept Medicare, Medicaid, and major health insurance plans for qualified patients.

Does my patient qualify for homecare?

  1. Patient must need, and a doctor must certify that patient needs, one or more of the following:
    • Intermittent skilled nursing care
    • Physical therapy
    • Speech-language pathology services
    • Continued occupational therapy
  2. A patient must be homebound, and a doctor must certify that the patient is homebound. 

An Individual does not have to be bedridden to be considered homebound. The Medicare definition states that occasional absences from the home for non-medical purposes (a trip to the barber, a walk around the block or a drive) will not necessarily disqualify a beneficiary from being classified as homebound. However, the absences must be infrequent or of a relatively short duration. The following is the Medicare definition of homebound.

Homebound means that leaving home is a taxing effort. A person may leave home for medical treatment or for short, infrequent absences for non-medical reasons, such as a trip to attend religious services. In certain circumstances you can still receive home health care even if you attend adult day care.

How do I refer a patient to Dimensions Home Health Care?

There are three easy methods to refer a patient to Dimensions Home Health Care

  • Call our agency (847-360-7660 or toll free at 866-706-9212) with orders and speak to a representative 24 hours a day, 7 days a week.
  • Click here to complete and to electronically submit a referral.
  • Complete the Physician Referral Form and fax to 847-360-8411.

A comprehensive assessment and evaluation will be completed by a registered nurse who will then admit your patient to our care. A plan of treatment will be sent to the physician’s office for review and signature.

What services does Dimensions Home Health Care provide?

Our Clinical team assesses the patient in their home and we work with the physician to develop the paln of care. Alternate Solutions provides the following care:

What if my patient needs on-going care?

If your patient requires continued care, at the end of the 60-day certification period, our agency will deliver an updated plan of treatment.

Rehab at Home: Rehab at Home is an innovative rehabilitation program for persons recovering from orthopedic injuries, as well as hip, knee or shoulder joint replacement.

  • Program Expert: A Physical Therapist will be your single point of contact and will work with your practice to develop customized protocols.
  • Next Day Service: Therapy services available the day after patient is discharged, including weekends.
  • Quality Patient Care:
    • Therapy Frequency: High frequency therapy (5x in first week) is available.
    • Quality Outcomes: Dimensions Home Health Care consistently ranks in the top 20% of the nation. 
  • Communication:
    • Patient Information Packet: Mailed to the patient 2 weeks prior to surgery explaining the Rehab at Home Program with educational brochures.
    • Pre-surgery Phone Call: A Program Liaison addresses questions and conducts a home safety evaluation via phone to ensure the home environment is optimal for patient’s post-operative return home.
